Mitral valvotomy during pregnancy; eighteen cases.

چکیده
Mitral stenosis is especially common in women of child bearing age. Of our first 350 mitral valvotomies, 64 per cent. were women below the age of 45 and i8 of these women were pregnant. There are several reports in the literature of the value of mitral valvotomy during pregnancy.6, 7, 8. 11, 12, 13, 20, 21, 25 Organic heart disease, especially mitral stenosis, is one of the leading causes of maternal morbidity and mortality. Rheumatic heart disease accounts for 90 per cent. of pregnant cardiac patients and in 75 per cent. of these there is involvement of the mitral valve,4 usually with mitral stenosis. The object of this communication is to present clinical details, criteria for selection of these cases, discussion about the optimum time for surgery, operative findings, immediate post-operative complications and correlation of operative findings with the post-operative progress.

Mitral Valvotomy in Relation to Pregnancy.
Conflicting views have been expressed regarding the advisability of mitral valvotomy in pregnancy. Little attention has been paid to the effect of pregnancy on the deterioration in cardiac status subsequent to mitral valvotomy. This study has been undertaken in an effort to clarify these two issues. Before June 1963, 903 patients under the care of one of us (J.F.P.) had mitral valvotomy. Of the...
متن کاملThe Association between B-type Natriuretic Peptide Family and Successful Percutaneous Transvenous Mitral Commissurotomy: A Systematic Review
B-type natriuretic peptide (BNP) level is known to increase in patients with rheumatic mitral stenosis. In this systematic review, we aimed to discuss the possible association between plasma BNP level and the success rate of percutaneous transvenous mitral commissurotomy. PubMed and Scopus databases were searched systematically, using the following key terms: “B-type natriuretic peptide” OR “BN...
متن کاملPercutaneous transluminal balloon dilatation of the mitral valve in pregnancy.
Pregnancy can cause life threatening complications in women with mitral stenosis, and there is a substantial risk of fetal death if valvotomy under cardiopulmonary bypass is required. A patient is described in whom pulmonary oedema developed after delivery of her first child by caesarean section 13 months previously. Subsequent cardiac catheterisation showed severe mitral stenosis (valve area 0...
متن کاملPreoperative clinical predictors of long term survival in mitral stenosis: analysis of 200 cases followed for up to 27 years after closed mitral valvotomy.
Two hundred patients aged 17-40 years undergoing closed mitral valvotomy during 1955-60 were studied by actuarial survival analysis. The period of follow up was 22-27 years. The following preoperative features were found to be independent predictors of long term survival: sinus rhythm (p less than 0.05); pulmonary arterial pressure below systemic pressure (p less than 0.01); absence of congesti...
متن کاملValvotomy in calcific mitral stenosis.
The results of transventricular mitral valvotomy in 50 cases of heavily calcified mitral stenosis are presented. The mortality and quality of results in patients undergoing a first valvotomy were more satisfactory than in patients undergoing a second operation. Associated mitral incompetence has an adverse effect on the results, particularly after a second operation. As a result, it is suggeste...
